IAAPA HALL OF FAME AWARD - Nomination Form

Criteria for IAAPA Hall of Fame Award

To be considered for the IAAPA Hall of Fame, a nominee must:

Have been associated with the attractions industry for a minimum period of 10 years.
Have been associated with a theme or amusement park, waterpark, family entertainment center, zoo, aquarium, museum, science center, casino, resort or other attraction, manufacturer, supplier, designer, or attractions industry consultant.
Have made a significant contribution to the worldwide attractions industry that has provided innovation, education, leadership, and/or influence both inside her/her organization AND within the industry in a manner that will continue to be of value in the future. Examples include contributions to the attractions industry at large such as: 1) Literary contribution 2) Design or invention of material equipment, processes, or technical services 3) Innovation in conjunction with attractions, rides, exhibitions, entertainment, food, merchandise, games, engineering, or safety.

I.	Nominee Summary (Required): Please review the award criteria. State concisely (150 words or less) in the space provided below why the nominee should be selected for induction into the IAAPA Hall of Fame. (This statement will form the basis for the award presentation should the nominee be selected.) «
II.	Description of Nominee's Industry Contribution and Involvement (Required): On a separate, typewritten document, summarize the nominee's activities and contributions within the attractions industry in no more than three pages. III.	Timeline (Required): On a separate, typewritten document, please include a timeline of the significant dates related to the activities or events cited in the nomination. This will help provide context for the nominee’s contribution(s). Maximum upload size is 10 MB.
IV.	Supporting Materials (Optional): On a separate, typewritten document, provide URL links to articles or web-based material that will help the committee validate the nominee’s contribution to the industry. Materials referenced should be written by primary sources (i.e., people who know/knew the nominee) or news articles from the period. Please do not include references to information on Wikipedia or other non-verified sources. Please provide a brief description of what is included in each link. Alternatively, attach copies of any relevant newspaper articles, magazine articles, or excerpts from books that document and validate the nominee’s industry contributions.
